What is PICC ?

Peripherally Inserted Central Catheters (PICCs) are long, flexible central venous catheters inserted via peripheral veins (typically the basilic, brachial, or cephalic) and advanced to the lower third of the superior vena cava (SVC).

Clinical Indications

Prolonged intravenous therapy (>14 days)

Irritant or vesicant infusions

Parenteral nutrition (e.g., TPN with high osmolarity)

Home infusion therapy

Frequent blood draws or blood transfusions

Frequent or difficult venous access

Benefits

Central Venous Access Without Surgical Placement

Reduced Complication Risk Compared to Other CVCs

Long-Term Dwell Time

Cost-Effective

Lower Infection Rates Than Non-Tunneled CVCs

Easy Removal

Outpatient Compatibility

Complications

Catheter-Related Bloodstream Infections (CRBSI)

Line Occlusion

Phlebitis and Infiltration

Thrombosis

Malposition or Migration

Relative contraindications

Known DVT or thrombosis in target vein

Active infection at insertion site

Severe coagulopathy (unless corrected)

Limited venous access due to previous PICCs or dialysis fistulas

Renal patients with limited vein options (preserve for future

Conclusion

PICC lines are a versatile, safe, and efficient central access option when inserted by trained clinicians using ultrasound guidance and strict sterile technique. They are particularly advantageous in patients needing medium- to long-term therapy, those with difficult access, and those transitioning to outpatient care.
