Cytotoxic IV fluids include medications and solutions that are:

  • Toxic to cells

  • Vesicants or strong irritants

  • Capable of causing severe tissue damage if extravasation occurs

  • Certain antiviral and immunosuppressive therapies

  • High-risk investigational medications

 

Because of their properties, cytotoxic agents should not be administered through standard peripheral IVs except in rare, tightly controlled circumstances.

What IV Access Should Be Used for Cytotoxic Fluids?​

Central Venous Access (RECOMMENDED)

 

For cytotoxic IV fluids, central access is the standard of care.

Appropriate Access Options:

  • PICC lines

  • Tunneled central venous catheters

  • Implanted ports

  • Non-tunneled central lines (short-term inpatient use)

No Peripheral IVs

(Generally NOT Recommended)

Peripheral IVs are high risk for cytotoxic infusions due to:

  • Small vein size

  • Higher infiltration and extravasation rates

  • Limited blood flow for rapid dilution
